There are many surgeons in the USA who would be able to perfore this type of procedure. You should research surgeons who specialize in skin grafting, pediatrics, and plastic and reconstructive surgery. It may require more than one surgeon to get the job done right (specializing in different areas such as hands/ facial). I would start with major medical institutions such as USC university hospital, John Hopkins, Ect. As for cost, it will most likely be expensive, but it is hard to tell until you have gone through a consultation.
